ABSTRACT

INTRODUCCIÓN. La enfermedad relacionada con IgG4 es una patología fibroinflamatoria multiorgánica, de origen desconocido, que simula trastornos malignos, infecciosos e inflamatorios. Los criterios del American College of Rheumatology y la European League against Rheumatism 2019, son útiles para el diagnóstico diferencial de ésta enfermedad cuando se no se cuenta con evidencia de inmunoglobulina G4 en sangre. CASO CLÍNICO. Paciente hombre de 45 años de edad, nacido en Ambato-Ecuador, con ingreso en noviembre del 2017, en emergencias del Hospital de Especialidades Carlos Andrade Marín, con presencia de tos con hemoptisis leve, febrícula, astenia, pérdida de peso e hiporexia de dos semanas de evolución. Se realizó múltiples exámenes, tras observar infiltrados pulmonares intersticiales, con elevación de inmunoglobulina G en suero, negativas para malignidad; se sospechó de enfermedad relacionada a inmunoglobulina G4. Se ampliaron los estudios para descartar otras patologías más prevalentes y cuyo diferencial es primordial. Se inició tratamiento con prednisona y micofenolato con buena respuesta clínica; durante dos años. DISCUSIÓN. La evidencia científica registró que el hallazgo más importante en la enfermedad relacionada con inmunoglobulina G4 fue un aumento de sus niveles séricos. La recurrencia de la enfermedad en un órgano afectado o la aparición de un nuevo órgano involucrado pudo conducir al diagnóstico en el caso presentado. CONCLUSIÓN. La enfermedad relacionada con inmunoglobulina G4 al ser una patología heterogénea, inmunomediada, al simular otras afecciones puede retrasar el diagnóstico; se debe tener una alta sospecha clínica, si al excluir otros procesos infecciosos, autoinmunes y/o eoplásicos, hay evidencia de patología fibroesclerosante multiorgánica sin causa establecida.


INTRODUCTION. IgG4-related disease is a multiorgan fibroinflammatory pathology of unknown origin that mimics malignant, infectious, and inflammatory disorders. The criteria of the American College of Rheumatology and the European League against Rheumatism 2019 are useful for the differential diagnosis of this disease when there is no evidence of immunoglobulin G4 in blood. CLINICAL CASE. 45-year-old male patient, born in Ambato-Ecuador, with admission in November 2017, in the emergency room of the Hospital de Especialidades Carlos Andrade Marín, with the presence of cough with mild hemoptysis, fever, asthenia, weight loss and hyporexia of two weeks of evolution. Multiple tests were performed, after observing interstitial pulmonary infiltrates, with elevated serum immunoglobulin G, negative for malignancy; immunoglobulin G4-related disease was suspected. Studies were extended to rule out other more prevalent pathologies whose differential is paramount. Treatment with prednisone and mycophenolate was started with good clinical response; for two years. DISCUSSION. The scientific evidence recorded that the most important finding in immunoglobulin G4-related disease was an increase in its serum levels. Recurrence of the disease in an affected organ or the appearance of a new involved organ could have led to the diagnosis in the presented case. CONCLUSION. Immunoglobulin G4-related disease, being a heterogeneous, immune-mediated pathology, by simulating other conditions may delay diagnosis; a high clinical suspicion should be maintained if, when other infectious, autoimmune and/or neoplastic processes are excluded, there is evidence of multiorgan fibrosclerosing pathology without established cause.

Abstract Introduction: Warthin tumors are the second most common benign tumors of the parotid gland. We examined the clinical features of Warthin tumors in our hospital, and analyzed the consistency within the literatures. Objective: The aim of this study is to analyze the clinical features of Warthin tumors in our 10-year experience of 118 Warthin tumors undergoing surgery at a single institute. Methods: From December 2006 to December 2016, 110 patients who underwent surgical treatment for Warthin tumors were identified based on their medical records. Results: A total of 118 parotid gland operations were performed in 110 patients. Almost 90% of Warthin tumors were found in males, and average patient age was 66.1 ± 6.1 years. The prevalence of smoking history was 89.1% (98/110). Eight patients (7.3%) had bilateral Warthin tumors. Seventy-seven lesions (65.3%) were located in the parotid tail portion, followed by 34 lesions in the superficial lobe (28.8%) and 7 lesions in the deep lobe (5.9%). Conclusion: We determined the appropriate extent of surgery depending on the fine needle aspiration cytology and tumor location by computed tomography scans. Partial facial dysfunction after the operation was detected in 12 cases, and facial nerve function recovered within 3 months. Only one patient experienced a recurrence, and was disease free after the re-operation. We suggest that our treatment algorithm, depending on the location of tumors and the result of fine needle aspiration cytology, can be useful to determine the appropriate extent of surgery for Warthin tumors.


Resumo Introdução: Os tumores de Warthin são os segundos tumores benignos mais comuns da glândula parótida. Avaliamos as características clínicas dos tumores de Warthin em nosso hospital e analisamos a consistência com a literatura. Objetivo: Analisar as características clínicas dos tumores de Warthin em nossa experiência de 10 anos de 118 tumores de Warthin submetidos a tratamento cirúrgico em um único instituto. Método: De dezembro de 2006 a dezembro de 2016, 110 pacientes que receberam tratamento cirúrgico para tumores de Warthin foram identificados com base em seus prontuários médicos. Resultados: Foram feitas 118 cirurgias na glândula parótida em 110 pacientes. Quase 90% dos tumores de Warthin foram encontrados em homens e a média da idade dos pacientes foi de 66,1 ± 6,1 anos. A prevalência de tabagismo foi de 89,1% (98/110). Oito pacientes (7,3%) tinham tumores de Warthin bilaterais na glândula parótida. Das lesões, 77 (65,3%) localizavam-se na porção da cauda da parótida, seguidas por 34 no lobo superficial (28,8%) e 7 no lobo profundo (5,9%). Conclusão: Determinamos a extensão apropriada da cirurgia de acordo com a punção aspirativa com agulha fina e localização do tumor por tomografia computadorizada. Disfunção facial parcial após a cirurgia foi detectada em 12 casos e a função do nervo facial foi recuperada em 3 meses. Apenas um paciente apresentou recidiva e ficou livre da doença após reoperação. Sugerimos que nosso algoritmo de tratamento, a depender da localização dos tumores e do resultado da PAAF, pode ser útil para determinar a extensão apropriada da cirurgia para os tumores de Warthin.

Abstract Introduction: Submandibular glands are exposed to many effects due to diseases and therapeutic interventions. A study evaluating the effect of submandibular gland dysfunction on the parotid gland has not been presented in the literature. Objective: The aim of this study was to evaluate the histopathological changes in the parotid gland following submandibular gland failure. Methods: Three groups of seven randomly selected female New Zealand rabbits weighing 2500-3000 g were studied. Unilateral and bilateral submandibular glands were removed in Groups 1 and 2, respectively. No procedure was performed in Group III, the control group. The parotid glands were removed 30 days later. Histological parameters were evaluated and graded between 0 (none) and 3 (severe). Differences between groups were compared using the Mann-Whitney U test. Results: Mean mucus accumulation in acinar cells was 2.57 ± 0.53 and 1.71 ± 0.75 in Groups 1 and 2, respectively (p < 0.05). This value was 0.57 ± 0.53 in Group 3, which was significantly lower than in Groups 1 and 2 (p < 0.05). Mean dilatation of the intercalated ducts' lumen was 1.28 ± 0.48 and 1.57 ± 0.53 in Groups 1 and 2, respectively (p > 0.05). This value was 0.28 ± 0.48 in Group 3, which was significantly lower than in Groups 1 and 2 (p < 0.05). Mean mucus accumulation in the intercalated ducts' lumen was 2.00 ± 0.81 and 1.00 ± 0.57 in Groups 2 and 3, respectively (p < 0.05). Conclusion: The findings of this study indicate that only 1 month after submandibular gland failure, the parotid glands exhibit significant changes.


Resumo Introdução: As glândulas submandibulares estão expostas a muitos efeitos causados por doenças e intervenções terapêuticas. Estudos que avaliam o efeito da disfunção da glândula submandibular na glândula parótida ainda não foram reportados na literatura. Objetivo: O objetivo deste estudo foi avaliar as alterações histopatológicas na glândula parótida após insuficiência da glândula submandibular. Método: Três grupos de sete coelhas fêmeas da raça Nova Zelândia, selecionadas aleatoriamente, pesando entre 2.500 e 3.000 gramas foram estudadas. As glândulas submandibulares unilaterais e bilaterais foram removidas nos Grupos 1 e 2, respectivamente. Nenhum procedimento foi realizado no Grupo III, o grupo controle. As glândulas parótidas foram removidas 30 dias depois. Os parâmetros histológicos foram avaliados e classificados entre 0 (nenhum) e 3 (grave). As diferenças entre os grupos foram comparadas usando o teste U de Mann-Whitney. Resultados: O acúmulo médio de muco nas células acinares foi de 2,57 ± 0,53 e 1,71 ± 0,75 nos Grupos 1 e 2, respectivamente (p < 0,05). Esse valor foi de 0,57 ± 0,53 no Grupo 3, significativamente menor do que nos Grupos 1 e 2 (p < 0,05). A dilatação média do lúmen dos dutos intercalados foi de 1,28 ± 0,48 e 1,57 ± 0,53 nos Grupos 1 e 2, respectivamente (p > 0,05). Introducción y Objetivo: La utilidad de la citología por punción aspiración con aguja fina en la valoración de tumores parotídeos es controvertida, ya que existe gran variabilidad en cuanto a la sensibilidad y especificidad en las distintas series publicadas. Nuestro objetivo es evaluar la utilidad de la punción aspiración con aguja fina (PAAF) de los tumores de glándula parótida en nuestro medio. Métodos: Se realizó un estudio retrospectivo y observacional con una muestra de 77 pacientes con tumores de glándula parótida, en el periodo 2000-2017 en dos centros hospitalarios de Montevideo, Uruguay, que fueron sometidos a punción aspiración con aguja fina preoperatoria y que posteriormente se correlacionó con el estudio anatomopatológico definitivo. Resultados: La edad media fue de 54 años con un rango comprendido entre los 14 y 87 años. El 56,8% eran mujeres y el 44,2% hombres. La sensibilidad de la punción aspiración con aguja fina para el diagnóstico de malignidad fue del 30% y la especificidad fue del 98,46%, con un valor predictivo positivo del 75% y un valor predictivo negativo del 90,14%. Conclusiones: La punción aspiración con aguja fina es un método diagnóstico muy útil a la hora de estudiar un tumor de glándula parótida, sobre todo por su valor predictivo negativo, lo que permite afirmar con alta probabilidad la benignidad del proceso.


Introduction and objectives: the usage of fine needle aspiration cytology in the evaluation of parotid gland tumors is controversial, since there is great variability in terms of sensitivity and specificity in different series published. Our objective is to evaluate the usage of fine needle aspiration cytology in the study of parotid gland tumors in our population. Methods: A retrospective study was conducted using a sample of 77 patients with parotid gland tumors between 2000-2017 in two hospital centers in Montevideo, Uruguay, who underwent preoperative diagnostic fine needle aspiration cytology and subsequently were treated with surgery and anatomopathological study. Results: The mean age was 54 years old, with interval between 14-87 years of age. 56.8% were female. The sensitivity of the fine needle aspiration cytology for the diagnosis of malignancy was 30%, and the specificity was 98.46% with a positive predictive value of 75% and a negative predictive value of 90.14% Conclusions: fine needle aspiration cytology is a very useful diagnostic method in the evaluation of parotid gland tumors, mainly due to its high negative predictive value.

Nosso objetivo é avaliar a utilidade da punção de aspiração de agulha fina de tumores da glândula parótida em nosso meio. Métodos: realizou-se estudo retrospectivo e observacional com uma amostra de 77 pacientes com tumores da glândula parótida, no período 2000-2017 em dois hospitais em Montevidéu, Uruguai, que foram sobmetidos a punção de aspiração com agulha fina pré-operatória e posteriormente correlacionou-se com o estudo anatomopatológico definitivo. A sensibilidade da punção de aspiração com agulha fina para o diagnóstico de malignidade foi de 30% ea especificidade foi de 98,46%, com um valor preditivo positivo de 75% e um valor preditivo negativo de 90,14%. Conclusões: A punção de aspiração com agulha fina é um método de diagnóstico muito útil na hora de estudar um tumor da glândula parótida, principalmente seu valor preditivo negativo, o que permite afirmar a benignidade do processo com alta probabilidade.

Resumo Esta revisão integrativa da literatura tem por objetivo identificar as principais lesões bucais que afetam pacientes pediátricos com HIV, bem como descrever o efeito da terapia antirretroviral altamente ativa (HAART) sobre essas lesões, comparando-a com a terapia antirretroviral (ART). Foi feita uma busca nas bases de dados PubMed e Scielo, seguindo critérios de inclusão e exclusão pré-determinados. Foram selecionados dezenove artigos científicos e extraídas as informações principais sobre prevalência e a frequência das manifestações bucais em pacientes pediátricos HIV positivos e o efeito da terapêutica aplicada. As lesões mais frequentes foram candidíase oral, gengivite, aumento das parótidas e eritema gengival linear. O uso da HAART mostrou diminuir a prevalência das manifestações bucais nos pacientes pediátricos com HIV e ser mais eficaz que a ART. Os achados deste estudo sugerem que a manifestação bucal mais frequente em pacientes pediátricos com HIV é a candidíase oral, seguida de alterações como gengivite e aumento das glândulas parótidas. O uso de HAART parece reduzir a prevalência dessas lesões orais, apresentando resultados mais eficazes que os da ART.


Abstract This integrative literature review aims to identify the main oral lesions affecting pediatric patients with HIV, and describe the effect of highly active antiretroviral therapy (HAART) on these injuries, comparing it to antiretroviral therapy (ART). A search was conducted in PubMed and Scielo databases, following predetermined inclusion and exclusion criteria. 19 papers were selected and the main information on the prevalence and frequency of oral manifestations in HIV-positive pediatric patients and effect of therapy applied were extracted. The most frequent injuries were oral candidiasis, gingivitis, parotid gland enlargement and linear gingival erythema. The use of HAART shown to reduce the prevalence of oral manifestations in pediatric patients with HIV and be more effective than ART. The findings of this study suggest that the most frequent oral manifestation in HIV-infected children is oral candidiasis, followed by changes such as gingivitis and enlargement parotid glands. The use of HAART appears to reduce the prevalence of these oral lesions, showing more effective results than ART.

Abstract Objective This study aimed to demonstrate the immunohistochemical changes associated with MMP-2 and type 1 collagen separately for the first time in the major salivary glands (the parotid, submaxillary, and sublingual glands) that occur with aging in mice. Material and Methods Fourteen Balb/c white mice (50-80 g) were used in this study. The animals were divided into two equal groups. Group I consisted of young animals (2-month-old) (n=7) and Group II consisted of older animals (18-month-old) (n=7). After routine histological follow-ups, Hematoxylin-eosin (H&E), Masson's Trichrome staining and immunohistochemical staining was performed for type I collagen and MMP-2. Results We observed that there were age-related decreases in the number of acinar cells, increase in eosinophilic zymogen granules in cells, collagen accumulation in fibrotic areas and dilatation in interlobular ducts. Also, while type I collagen and MMP-2 immunoreactivity were moderate in the salivary glands of the young mice, they were high in the salivary glands of the old mice (p=0.001). In the H-score assessment, MMP-2 immunoreactivity was lower at a significant level in young mice than in old mice (p=0.001). Conclusions This study showed that anatomical, physiological and morphological abnormalities occur in all three major salivary glands as a natural consequence of aging.

Abstract Salivary gland tumors are relatively rare neoplasms, mostly located in the parotid gland, and few are malignant. Preoperative evaluation of salivary gland tumors includes fine needle aspiration cytology (FNAC). Objective The purpose of this study was to determine the importance of FNAC in the evaluation of rare salivary gland neoplasms. Material and Methods Four cases of rare salivary gland tumors were included, which were preoperatively assessed by clinical investigation, computed tomography, and FNAC. Results The presented cases include myoepithelial carcinoma, oncocytic carcinoma, undifferentiated lymphoepithelial carcinoma, and marginal zone lymphoma. Conclusion FNAC is a reliable diagnostic tool for common salivary gland neoplasms; however, rare tumors often represent diagnostic challenges. Clinical relevance In such rare tumors, the role of aspiration cytology may be limited to establishing the dignity of the lesion (benign/malignant). This knowledge enables the surgeon to choose the most appropriate therapeutic procedure. A definitive diagnosis of rare tumors (either epithelial or nonepithelial) is obtained by histological examination; cytology is limited in this regard due to overlapping features.

ABSTRACT Objective Fine-needle aspiration cytology is a valuable method for preoperative assessment of head and neck tumors. However, its accuracy in detection of salivary gland masses is controversial compared with other methods. The aim of this work was to evaluate the effectiveness and accuracy of fine-needle aspiration cytology (FNAC) in the diagnosis of parotid gland masses. Material and Methods Over a 10-year period, 126 parotid gland masses were resected. Retrospective chart reviews of 114 patients were performed. The results of FNAC and final histological diagnosis were compared and the accuracy of FNAC was determined. Results Final histological evaluation revealed 11 malignant tumors and 103 benign lesions. Pleomorphic adenoma was the most common neoplasm (63%), followed by Warthin’s tumor (17.5%). The sensitivity of FNAC in detecting malignant tumors was 73% and the specificity was 97%. Positive predictive value (PPV) was 73% and negative predictive value (NPV) was 97%. The overall accuracy of FNAC in detecting parotid masses was 95%. False-negative diagnosis was found in mucoepidermoid carcinoma, acinic cell carcinoma, and epithelial-myoepithelial carcinoma whereas there was false-positive diagnosis in cases of pleomorphic adenoma and normal parotid gland tissue. Conclusion FNAC is a reliable minimally invasive diagnostic method with a high sensitivity in diagnosis of lesions in parotid glands. The sensitivity of detection of malignant tumors in parotid glands was low due to the biopsy technique used, and depended on tumor location. Postoperative complications decreased after superficial parotidectomy.

Objetivo: Reportar casos de linfadenoma sebáceo, un tumor raro, localizado principalmente en la glándula parótida, con muy pocos casos descritos en la literatura. Casos clínicos: Presentamos 2 casos de esta rara afección, tratados en el Hospital Universitario Miguel Servet de Zaragoza, de enero de 2010 a diciembre de 2014. El diagnóstico, tanto por imagen como citológico, es difícil debido a su baja prevalencia y a la gran variedad de tumores de glándula parótida que existen. En los 2 casos presentados ha sido necesaria la exéresis para llegar al diagnóstico. El tratamiento curativo consiste en la exéresis completa de la tumoración.


Aim: To report a rare parotid tumour: sebaceous lymphadenoma, with very few cases reported in the literature. Cases report: We present 2 cases of lymphadenoma sebaceous of the parotid gland treated by surgery at University Hospital Miguel Servet of Zaragoza between January 2010 and December 2014. The diagnosis, both radiological and cytological, is difficult because of their low prevalence and the great variety of parotid gland tumors existent. In our 2 cases described, excision has been required for diagnosis. A complete excision of the tumor is required for curative treatment.

ABSTRACT

Objetivo: determinar la terapéutica más efectiva para el tratamiento del síndrome de Frey. Caso clínico: un paciente de sexo femenino, de 68 años de edad, se presentó a la consulta en el Servicio de Cirugía Bucomaxilofacial de la Unidad Asistencial Por + Salud Dr. César Milstein, por sudoración cervical molesta del lado izquierdo. Fue tratada con infiltración subcutánea de toxina botulínica por desarrollar síndrome de Frey posparotidectomía. Conclusión: actualmente, la infiltración subcutánea de toxina botulínica de tipo A constituye la primera opción de tratamiento, pues brinda resultados efectivos y seguros.

ABSTRACT

The article analyzes an experience in technical cooperation between Brazil and Andean countries in the form of the International Course in the Management of Human Resource Policies in Health. This exploratory documental study encompassed a number of Latin American countries whose institutions of higher education had partnerships with the Federal University of Rio Grande do Norte, mediated by the Pan American Health Organization Representation in Brazil. The course experience shows that fundamental values like ethics and solidarity are determinant to the success of technical cooperation processes.


Analisa a experiência de cooperação técnica entre o Brasil e os países da região andina, realizada por meio do Curso Internacional em Gestão de Políticas de Recursos Humanos em Saúde. Para tanto, foi efetivado estudo documental de caráter exploratório, envolvendo diversos países da América Latina, resultado de parcerias estabelecidas entre a Universidade Federal do Rio Grande do Norte e instituições de ensino superior dos países participantes. O estabelecimento das parcerias e a condução do processo foram mediados pela Organização Pan-americana da Saúde-Representação do Brasil. A experiência do curso possibilitou demonstrar que valores fundamentais como ética e solidariedade são determinantes para o êxito de processos de cooperação técnica.

Introducción: El síndrome de Sjögren (SS) es una enfermedad crónica, autoinmune, que afecta principalmente a las glándulas exocrinas lagrimales y salivales. En niños es una enfermedad rara. Objetivo: Presentar el caso de un adolescente con síntomas inespecíficos, en que la sospecha clínica hizo llegar al diagnóstico de SS. Caso Clínico: Paciente de 12 años, de sexo masculino, con historia de artralgias de 3 años de evolución y xeroftalmia dudosa. El examen físico mostró leve congestión conjuntival, boca seca e hiperlaxitud de rodillas. Pruebas de laboratorio: hemograma y VHS normales, anticuerpos antinucleares (+) > 60, Ro (+) > 60 U, factor reumatoideo (+) 160 UI/ml. Se sospechó SS y se completó el estudio: test de Shirmer, que determinó ojo seco leve; gammagrafía de las glándulas salivales, que mostró disfunción de las glándulas submaxilares y parotídeas; biopsia de glándulas salivales, que mostró focos de infiltrado linfoide acinar y periductal. Se confirmó SS y se inició tratamiento con prednisona 7,5 mg/día e hidroxicloroquina 200 mg/día y tratamientos locales, con buena respuesta. Conclusiones: Los criterios diagnósticos del SS en adultos identifican solo al 39% de los pacientes pediátricos por la baja frecuencia de síntomas de sicca. Aún no existen criterios diagnósticos validados para niños. Un buen diagnóstico permitirá aliviar los síntomas, evitar complicaciones y detectar enfermedades asociadas.


Introduction: Sjögren’s syndrome (SS) is a chronic autoimmune disease that primarily affects the lacrimal and salivary exocrine glands. In children, it is a rare condition. Objective: To present the case of an adolescent with non-specific symptoms, but with a clinical suspicion of SS. Case report: A male 12-year old patient, with history of arthralgias for 3 years and suspicion of xerophthalmia. Physical examination showed mild conjunctival congestion, dry mouth and hypermobility of the knees. Laboratory work: blood count and ESR were normal, antinuclear antibodies (+) > 60, Ro (+) > 60 U, and rheumatoid factor concentration (+) 160 IU/ml. SS was suspected, and a study was carried out: Schirmer test determined mild dry eye, salivary gland scintigraphy showed parotid and submandibular gland dysfunction, and salivary gland biopsy reported focal lymphocytic acinar and periductal infiltration. SS was confirmed and treated with prednisone 7.5 mg/day and hydroxychloroquine 200 mg/day, and local treatment, with good response. Conclusions: The diagnostic criteria for SS in adults identified only 39% of pediatric patients, due to the low frequency of sicca symptoms. Still there are no validated diagnostic criteria for children. A good diagnosis will alleviate symptoms, prevent complications and detect associated diseases.

Introdução: O lipoma é um dos tumores benignos mais comuns de origem mesenquimal e constitui-se de gordura. São raramente encontrados na região de cabeça e pescoço, principalmente na região parotídea. Objetivo: Relato de caso clínico de paciente com lipoma em região parotídea direita. Caso clínico: Paciente A.S.L, gênero feminino, melanoderma, 32 anos, procurou atendimento odontológico referindo-se ao aumento de volume em região parotídea direita. Ao exame clínico observava-se uma massa de consistência macia e superfície lisa com aproximadamente 3 centímetros de diâmetro e ausência de queixas álgicas por parte da paciente. Ao exame tomográfico observou-se imagem radiolúcida circunscrita próxima ao arco zigomático e lóbulo superior de parótida direita. A paciente foi orientada a realizar biópsia diagnóstica. O procedimento cirúrgico foi realizado sob anestesia geral e através de acesso pré-auricular, o laudo histopatológico confirmou a hipótese diagnóstica. Nova intervenção cirúrgica não foi necessária uma vez que a biópsia realizada foi excisional. Conclusão: Como o diagnóstico clínico dos casos de lipoma são fáceis de realizar dado as suas características específicas e aos auxílios de exames de imagens, a biópsia excisional torna-se o tratamento definitivo e eficaz, sendo o material enviado ao laboratório de análises clínicas apenas para corroboração da hipótese diagnóstica... Introduction: Lipoma is one of the most common benign tumors of mesenchymal origin and consists of fat. Are rarely found in the head and neck, especially in the parotid region. Objective: To report a case of a patient with a lipoma in the right parotid region. Case report: Patient A.S.L., female, melanoderma, 32-year-old, sought to dental care referring to the volume increase in the right parotid region. On clinical examination we observed a mass of soft consistency and smooth-surface with about 3 centimeters in diameter and no complaints of pain by the patient. The CT scan was observed circumscribed radiolucent image next to the right zygomatic arch and upper lobe of parotid gland. The patient was advised to diagnostic biopsy. The surgical procedure was performed under general anesthesia and through pre-auricular access, the histodiferenpathological report confirmed the hypothesis. New surgical intervention was not necessary as the excisional biopsy was performed. Conclusion: As the clinical diagnosis of lipoma cases are easy to perform given its specific characteristics and aid of imaging exams, excisional biopsy becomes final and effective treatment, the material was sent to the clinical laboratory only corroboration of the diagnosis... Pleomorphic adenoma of parotid gland, is a relatively common injury, with prevalence of approximately 70% of confirmed cases. Parotid gland is salivary gland most affected by pathology. Variety of histological structures found related to specific clinical behaviors cannot be proven yet. The aim of this paper is to present a pleomorphic adenoma, found in a female patient, leucoderma, in the fith decade of life, having its development there about two years. A sialocele parotídea é o extravasamento de saliva para os tecidos circunvizinhos à glândula parótida. Pode ser de causa idiopática, por trauma ou complicação pós-operatória do acesso retromandibular para abordagem do côndilo e ramo da mandíbula. O tratamento das fraturas de côndilo mandibular é controverso entre cirurgiões e a conduta cirúrgica vem trazendo excelentes resultados com mínimas complicações. Em casos de fratura com deslocamento medial, é absoluta a indicação de redução cirúrgica. Para tal, existem as opções de acesso intraoral, pré-auricular ou retromandibular. Este último tem a vantagem de proporcionar visão direta, porém apresenta risco de lesão ao nervo facial, formação de sialocele e fístulas salivares. The sialocele parotid is the extravation of saliva into the tissiues sorrounding the parotid gland. It may be idiopathic, trauma or postoperative complication of retromandibular approach to the condyle and mandibular ramus. The treatment of mandibular condyle fractures is controversial among surgeons, surgical management has brought excellent results with minimal complications. In cases of fracture with medial displacement is an absolute indication for surgical reduction. For this there are options: intraoral access, pre-auricular or retromandibular. The latter has the advantage of providing direct vision but has a risk of injury to the facial nerve, forming sialocele and salivary fistulas. The aim of this study was to evaluate the radioprotective effect of vitamin E on rat parotid glands by morphometric analysis. Sixty male rats were divided into 5 groups (n=6): control, in which animals received olive oil solution; olive oil/irradiated, in which animals received olive oil and were irradiated with a dose of 15 Gy of gamma radiation; irradiated, in which animals were irradiated with a dose of 15 Gy gamma radiation; vitamin E, which received α-tocopherol acetate solution; vitamin E/irradiated, which received α-tocopherol acetate solution before irradiation with a dose of 15 Gy gamma rays. Half of the animals were euthanized at 8 h, and the remaining at 30 days after irradiation. Both parotid glands were surgically removed and morphometric analysis of acinar cells was performed. Data were subjected to two-way ANOVA and Tukey's test (α=0.05). Morphometric analysis showed a significant reduction in the number of parotid acinar cells at 30 days in olive oil/irradiated and irradiated groups. In groups evaluated over time a significant reduction was shown at 30 days in olive oil/irradiated and irradiated groups, indicating that ionizing radiation caused tissue damage. The vitamin E/irradiated group presented more acinar cells than the irradiated group, but no statistically significant difference was observed (p>0.05). In conclusion, vitamin E seems to have failed as a radioprotective agent on acinar cells in rat parotid glands.


O objetivo neste estudo foi avaliar o efeito radioprotetor da vitamina E sobre glândulas parótidas de ratos por meio de análise morfométrica. Sessenta ratos machos foram divididos em cinco grupos: controle, no qual os animais receberam solução de óleo de oliva; óleo de oliva irradiado, em que os animais receberam óleo de oliva e foram irradiados com uma dose de 15 Gy de radiação gama; irradiado, em que os animais foram irradiados com uma dose de 15 Gy de radiação gama; vitamina E, no qual receberam solução de acetato α-tocoferol; vitamina E irradiado, os quais receberam solução de acetato de α-tocoferol antes da irradiação com uma dose de 15 Gy de radiação gama. Metade dos animais foi eutanasiada em 8 h, e o restante aos 30 dias após a irradiação. Ambas as glândulas parótidas foram removidas cirurgicamente e análise morfométrica das células acinares foi realizada. Os dados foram submetidos à Análise de Variância com 2 fatores e teste de Tukey (α=0,05). A análise morfométrica mostrou uma redução significativa no número de células acinares da glândula parótida aos 30 dias nos grupos óleo irradiado e irradiado. Nos grupos avaliados ao longo do tempo uma redução significativa foi mostrada aos 30 dias nos grupos óleo irradiado e irradiado, indicando que a radiação ionizante causou danos teciduais. O grupo vitamina E/irradiado apresentou mais células acinares que o grupo irradiado, mas diferença estatisticamente significante não foi observada. Em conclusão, a vitamina E parece ter fracassado como um agente radioprotetor nas células acinares das glândulas parótidas de ratos.

A histologia do parênquima das glândulas parótidas pode estar alterada por condições locais ou sistêmicas, patológicas ou não patológicas. OBJETIVO: Este trabalho objetiva destacar a lipossubstituição das parótidas, uma condição não patológica pouco conhecida. MÉTODO: Em um estudo retrospectivo, reuniu-se a casuística de seis pacientes que se apresentaram clinicamente com aumento de volume das parótidas, mas que, após ampla investigação, não se chegou a um diagnóstico. RESULTADOS: Por meio da análise retrospectiva de seus prontuários, foi constatada, em todos, lipossubstituição das parótidas, em estudo de ressonância magnética. CONCLUSÃO: Essa condição, apesar de fisiológica e esperada com o envelhecimento, pode relacionar-se com o aumento clínico e radiológico dessas glândulas.


Parotid gland parenchyma histology may be altered by local or systemic, pathological or nonpathological conditions. OBJECTIVE: This paper aims to highlight the fatty degeneration of the parotid gland, a not well-known non-pathological condition. METHOD: In a retrospective study, we collected a series of 6 patients which presented a swelling of the parotid, but even after extensive research, a diagnosis was not reached. RESULTS: Through the retrospective analysis of records we found fatty degeneration of the parotid in an MRI scan of all patients. CONCLUSION: This condition, despite being physiological and expected with aging, may be related to clinical and radiological swelling of these glands.

Obesity is a major health problem that affects up to 30% of adult and has been linked to an increase in dietary intake, especially fat intake and a sedentary lifestyle. In addition, it is a well-known aggravating factor in the pathology of many organs. The aim of the study was to investigate the histological and immunohistochemical changes in the parotid gland of the rats maintained on rich-fat diet. Sixteen adult male albino rats were divided equally into a control group I and a high-fat diet group II. Rats in group II were fed a high-fat diet for 3 months. At the end of the experimental period, blood samples were collected for detection of total cholesterol and triglyceride. Half of the parotid samples were processed for light microscopic examination, whereas the other half were prepared for electron microscopic examination. Paraffin sections were stained with H and E, Mallory trichrome, and also with alpha smooth muscle actin [alpha-SMA]. Specimens from group II showed disarrangement of the acinar cells, cytoplasmic vacuolization, and nuclear irregularity. Marked fibrosis between serous acini and intense cellular infiltration were observed. An apparent increase in the immunoreaction for alpha-SMA was found at the periphery of the acini and interlobular ducts. The results of the present study suggest that there is a significant relationship between fat-rich diet intake and structural changes in the parotid gland, such as massive fibrosis, numerous dilated blood vessels, and intracellular vacuolization with lipid droplets

ABSTRACT

Warthin's tumor also known as papillary cystadenoma lymphomatosum is a common benign salivary gland neoplasm that occurs exclusively in parotid gland. Rarely, the tumor can undergo carcinomatous or lymphomatous transformation of epithelial or lymphoid component, respectively. Herein, we describe a case of 55-year-old female who had undergone parotidectomy for a rapidly growing tumor in the right parotid gland. The case was diagnosed as mucoepidermoid carcinoma developed in a setting of Warthin's tumor based on the histomorphology, special staining characters, and immunohistochemical findings. The pathogenesis and differential diagnoses of such rare malignancy has been discussed briefly.
